Provides case management for assigned grant program; performs client intake, determines eligibility, identifies needs, and makes referrals for comprehensive wrap-around services. Maintains case files; processes financial assistance; provides training.
Screens all patients for third party eligibility; providing assistance and follow up in completing applications, ensuring referrals are processed appropriately, and educating individuals on the importance of alternate resources.
Provides direct services to beneficiaries both in Kodiak and assigned villages, facilitates coordination of care with other KANA providers, provides clinical supervision of Behavioral Health Aides in assigned villages.
